Department/Committee-Select the name of the Department project/event represented.
Type of Activity-List what you did.
Date of Activity-Select the date you participated/completed the activity.
Description of Project/Activity-Please write a short synopsis of the activity, the benefits of the activity/project to the club and tothe community.
Publicity & Promotion Used-List how you heard about this project/activity.
Hours Given to Activity-List all of the hours YOU gave toward that particular project. You may include time spent collecting, sorting, tabulating items, etc.
Total Money Donated-List all monies given directly to the club or organization for the project.
$ Amount of Items Donated-List dollar amount of any goods or services donated for the project.
